CONCEPT CARE DISCOUNT PLAN is our comprehensive eye care plan offered to families and small businesses (including employers, employees and their family members). If you do not currently have a vision care plan, or cannot afford another, please consider this one. You may be referred to an eye specialist of your choice for some medical services. Contact your major medical insurance company directly for information regarding benefits coverage if it applies.
What's Covered?
- Our routine eye exam for glasses and/or contact lenses. One annual per covered person.
- Discounts on eyeglass frames, lenses and contact lenses.
- Savings on medical visits and other eye care related procedures:
- Treatment for pink eye, allergy eyes, glaucoma, dry eyes etc. Includes follow-up care.
- Foreign body removal: objects such as metal or wood chips, misdirected eye lashes, etc. This can often be done in our office.
Comprehensive eye exams include:
- An eyeglass perscription
- Glaucoma screening
- Cataract evaluation
- Pupillary dilation (if warranted)

We accept most insurance plans -vision and medical. Co-payments for insurance plans are due at the time of service. A deposit is required to order eyewear and/or contact lenses unless the insurance coverage dictates otherwise. |
